Once you have been referred to our hospital, a clinical intake coordinator will perform an on-site assessment. This will give you an opportunity to learn more about the services we provide. Our admissions department will then coordinate your transfer to our facility including working with your insurance provider, physician, and discharge planner.
A referral can be made from the following sources:

- Family or friends
- Home health agencies
- Long-term acute care hospitals
- Skilled nursing facilities
